We are seeking an ambitious individual to join our Social Care Community Engagement Team as a Care Professional. In this role, you will play a vital part in implementing "Living Well" – a model that focuses on real conversations with individuals to create better outcomes.

Your Key Responsibilities

You will need to develop strong communication skills, record keeping, and keyboard skills, along with experience of working with vulnerable people who require care and support. You should have a commitment to excellent customer service and an ability to manage a high turnover of work in an organised and efficient way. You will have a strong commitment to multi-disciplinary working and be familiar with the departments' eligibility criteria.

Depending on your skills and experience, you may be offered the post of Supported Assistant Practitioner (Grade F) and you would be expected to complete our 'Assistant Practitioner development programme' in an agreed timescale.
