About the Role

Hales Home Care is seeking experienced Care Workers to join our Reablement Intervention Care Team in Norwich/East Norfolk.

This is an exciting opportunity to work in a dynamic team, providing person-centred care and support to individuals in their own homes.

Key Responsibilities
  • Work in teams of two to support rapid hospital discharges and provide care and support to enable individuals to regain independence and achieve their goals for independent living.
  • Conduct dynamic risk assessments and deliver care with an outcomes-focused attitude.
  • Liaise with other health professionals and signpost service users and family members to additional support, equipment, and services.
Requirements
  • Be available to work 7 days per week on a 4 on/3 off shift pattern.
  • Have a full driving licence and access to your own vehicle.
  • Have previous experience in providing person-centred care.
