Job Summary

The Humbleyard Practice is seeking an experienced healthcare professional to join our nursing team in a role that combines outreach visiting of housebound patients, care home support, and in-practice clinics. This is an exciting opportunity to work in a flexible environment where we can work with you to agree a working pattern that suits your needs.

Main Responsibilities

You will manage a caseload of housebound patients, providing advice and assistance with their continuing medical needs. You will undertake acute visits on a daily basis, providing care for patients from initial history taking, clinical assessment, and diagnosis. You will liaise with patients' GPs, as required, and implement treatment plans, demonstrating safe and effective clinical decision-making and holistic care.

About the Role

The Humbleyard Practice is a large, semi-rural practice delivering primary care services to over 22,000 patients. We work as one caring and supportive team across three sites, and we pride ourselves on providing patient-centred care to our local population. Person Specification

Essential

  • NMC or HCPC registration
  • Knowledge of the needs of patients with long-term conditions
  • Excellent communication skills
  • Ability to work collaboratively with the practice team to meet the needs of patients, supporting the delivery of policy and procedures

  • Independent Prescribing qualification
  • Post-registration qualification in long-term conditions management
  • MSc in Health & Social Care
