About the Role

Magdalen Medical Practice is seeking to recruit a care coordinator to join their busy team on a full or part-time basis.

The post-holder will provide extra time, capacity and expertise to our practice team to help us improve the care provided to our patients.

Main Responsibilities

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Experience of completing holistic person-centred care planning assessments.
  • Strong listening and communication skills.

About the Practice

Magdalen Medical Practice has a rich history dating back to the First World War. We have maintained the best traditions of family medicine while embracing important modern developments.

Our practice is spacious, comfortable and modern. We have a car-park, easy access via automatic main entrance doors, hearing loop and facilities for the disabled. Our catchment area includes Catton, Sprowston, Spixworth and Heartsease.
