Job Overview

We are seeking an experienced and skilled professional to join our Care Home Support Team as a Senior Mental Health Practitioner for GP. This is a unique opportunity to work with a dedicated team of healthcare professionals to provide high-quality care to residents in nursing and residential care homes.

Main Responsibilities
  • Triage new referrals to the Postural Management Team and provide accurate feedback to the relevant therapist regarding interventions and any clinical observations.
  • Support the initial assessment and review of residents requiring support with seating, bed positioning, specialist chairs, and positioning cushions.
  • Work collaboratively with staff from all disciplines to ensure good communication and effective processes and procedures are maintained across the service.
  • Provide administrative support to the Care Home Support Team and undertake any administrative duties associated with the service as requested by the team.
  • Liaise effectively and professionally with all levels of staff, external organisations, and the public, dealing with confidential information in an appropriate manner.
Requirements
  • Education to GCSE standard in English and Maths as a minimum.
  • Evidence of continuing professional development.
  • NVQ 3 in a relevant subject or equivalent experience.
  • A working knowledge of Microsoft products, including Excel.
