Job Summary

We are seeking a highly motivated and compassionate Registered Mental Health Nurse to join our established Multidisciplinary team as part of our Community Transformation programme. The successful candidate will work with a diverse range of clinical professionals to provide recovery-focused treatment and support to individuals with mental health disorders, empowering them to lead fulfilling lives.

Main Responsibilities
  • Work collaboratively with a Multidisciplinary Team to provide holistic care to individuals experiencing mental health issues and support their recovery journey.
  • Act as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, ensuring comprehensive assessments and personalized care plans.
  • Support carers and system partners, promoting a collaborative approach to mental health care.
  • Undertake Mental Health Nursing Needs Assessments/Care Act Assessments, as required, and complete Social Circumstance reports and attendance at Mental Health Review Tribunals.
  • Administer prescribed depot injections/medication monitoring and Clozapine Clinic, as required.
  • Conduct comprehensive assessments, formulating personalized care plans that reflect the needs of service users, their families, and wider support networks.
  • Provide psycho-social interventions, education, and support to develop service users' and carers' understanding of mental illness, aiming to prevent relapse and admission.

  • Professional registration.
