Senior Community Mental Health Practitioner

Essex Partnership University NHS Foundation Trust is seeking a Registered Mental Health Nurse to join our established Multidisciplinary team as part of our Community Transformation programme. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders.

We are looking for a positive team player who has a special interest in working compassionately with individuals and their wider support networks.

Main duties of the job

Working as part of an established Multi-Disciplinary Team within the Community Mental Health setting: providing holistic care to those who are experiencing mental health issues and supporting their recovery journey.

Acting as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users.

Undertaking Mental Health Nursing Needs Assessments/Care Act Assessments, as required.

Completing Social Circumstance reports and attendance at Mental Health Review Tribunals.

Administration of prescribed depot injections/ medication monitoring and Clozapine Clinic (as required)

Undertaking comprehensive assessments: formulating personalised care plans which reflect the needs of the service user, their family and wider support networks

Conduct risk assessment of the individual and of the potential hazards in the service users environment

Providing psycho-social interventions, education and support to develop service users and their carers understanding of mental illness with the aim to prevent relapse and admission

Job responsibilities

Person Specification
  • Valid UK driving licence
  • Access to own car for work
Professional
  • Professional registration
  • Evidence of professional development
Experience
  • Minimum of 12 months Band 5 experience
  • Previous Community Mental Health experience
