Job Title: Adults Mental Health Senior Practitioner

We are seeking a highly skilled and compassionate Adults Mental Health Senior Practitioner to join our established Multidisciplinary Team in Essex Partnership University NHS Foundation Trust. As part of our Community Transformation programme, this role offers a unique opportunity to provide recovery-focused treatment and support to individuals with mental health disorders.

Main Responsibilities:
  • Working as part of an established Multi-Disciplinary Team within the Community Mental Health setting, providing holistic care to those experiencing mental health issues and supporting their recovery journey.
  • Acting as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, undertaking Mental Health Nursing Needs Assessments/Care Act Assessments as required.
  • Completing Social Circumstance reports and attending Mental Health Review Tribunals, administering prescribed depot injections/medication monitoring and Clozapine Clinic as needed.
  • Undertaking comprehensive assessments, formulating personalised care plans that reflect the needs of the service user, their family, and wider support networks.
  • Conducting risk assessments of the individual and potential hazards in the service user's environment, providing psycho-social interventions, education, and support to develop service users' and carers' understanding of mental illness.
Requirements:
  • Valid UK driving licence and access to own car for work.
  • Professional registration and evidence of professional development.
  • Minimum of 12 months Band 5 experience and previous Community Mental Health experience.
