About the Role

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our established Multidisciplinary Team within the Community Mental Health setting. As a Senior Community Mental Health Practitioner, you will play a vital role in providing holistic care to individuals experiencing mental health issues and supporting their recovery journey.

Key Responsibilities
  • Working as part of an established Multi-Disciplinary Team to provide comprehensive assessments and formulate personalized care plans that reflect the needs of service users, their families, and wider support networks.
  • Acting as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, providing psycho-social interventions, education, and support to prevent relapse and admission.
  • Supporting carers and system partners, undertaking Mental Health Nursing Needs Assessments/Care Act Assessments, and completing Social Circumstance reports and attendance at Mental Health Review Tribunals.
  • Administering prescribed depot injections/medication monitoring and Clozapine Clinic, as required.
Requirements
  • Valid UK driving licence.
  • Professional registration.
  • Minimum of 12 months Band 5 experience.
