About the Role

We are seeking a highly skilled and compassionate Registered Mental Health Nurse to join our Multidisciplinary team as a Senior Practitioner in Community Mental Health. This is an exciting opportunity to work with individuals who are experiencing mental health issues and support their recovery journey.

Main Responsibilities
  • Work collaboratively with a Multidisciplinary Team to provide holistic care to individuals experiencing mental health issues.
  • Act as a Care Co-ordinator/Lead Professional for a dedicated caseload of service users, developing and implementing personalized care plans.
  • Conduct comprehensive assessments, including Mental Health Nursing Needs Assessments and Care Act Assessments, to identify individual needs and develop effective care plans.
  • Administer prescribed depot injections and medication monitoring, as required, and participate in Clozapine Clinic.
  • Conduct risk assessments to identify potential hazards in the service user's environment and develop strategies to mitigate these risks.

Requirements
  • Valid UK driving licence and access to own car for work.
  • Professional registration and evidence of ongoing professional development.
  • Minimum of 12 months Band 5 experience and previous Community Mental Health experience.
