Occupational Therapist Specialist

We are seeking a highly skilled and motivated Specialist Occupational Therapist to join our dynamic Rapid Response Team (RRT) in Bexley. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals in need of urgent community response.
Key Responsibilities- Conduct thorough, holistic assessments of clients to identify their needs and develop effective care plans.
- Work closely with other healthcare professionals and social services to provide specialist Occupational Therapy advice and support.
- Develop and implement interventions to promote functional independence and safety, utilizing evidence-based practice and clinical reasoning skills.
- Collaborate with the team to ensure seamless care coordination and effective communication with clients, carers, and other stakeholders.
- Participate in multidisciplinary meetings and complex family conferences to ensure comprehensive care planning.

Requirements- Recognized Occupational Therapy Degree Qualification or equivalent.
- Health & Care Professions Council (HCPC) Registration.
- Post-registration experience as a Band 5 Occupational Therapist, with evidence of meeting Band 6 requirements.
- Experience working in a specialist Rapid Response Team or similar setting.
- Excellent professional and clinical knowledge and skills as an Occupational Therapist.
