About the Role

We are seeking a highly skilled and experienced Associate Case Manager to join our dynamic team at SNAP Care. As an Associate Case Manager, you will play a vital role in supporting our clients with acquired or traumatic brain injury, spinal injury, limb loss, dementia, and polytrauma across London and the Southeast Counties.

Key Responsibilities
  • Conduct thorough initial assessments of client care, rehabilitation, and case management needs under the supervision of the Lead Case Manager.
  • Develop and implement individualized rehabilitation goal-based programs to support clients in regaining independence and achieving their full potential.
  • Collaborate with therapists, medical practitioners, solicitors, social services, clients' employers, and schools to ensure comprehensive support for clients.
  • Act as an advocate for clients, ensuring they receive the right support in all aspects of their care plan.
Requirements
  • Minimum of 2 years' experience in a senior role within the disability/care sector, supervising people.
  • Excellent communication, IT, and written skills.
  • Ability to work independently and manage a caseload.
  • Driver with own car essential.
