We are seeking a highly skilled and experienced Qualified Social Worker to join our team at Polaris Children's Services. As a Qualified Social Worker, you will play a vital role in delivering innovative and outcomes-based services to children, young people, and families.
About the Role:
- Hold case responsibility and oversee the delivery of intervention services to children, young people, and families.
- Respond to referrals or service requests from Local Authority partners.
- Implement, monitor, and review service plans relating to the assessed needs of children, young people, and their families.
- Refer children, young people, and their families to appropriate specialist or universal services.
- Contribute to the provision of regular returns/contract monitoring reports to the customer.
- Support the Service Manager in delivering the Key Performance Indicators for the service.

Requirements:
- Holds a Social Work qualification recognised by Social Work England or equivalent body.
- Proven experience of working with children, young people, and their families with complex and multiple needs.
- A comprehensive working knowledge of all relevant legislation pertaining to children's services and fostering.
- Ability to work effectively in partnership with Local Authorities and partner agencies.
