About the Role

We are seeking a highly skilled and compassionate Social Worker to join our team at Nottingham University Hospitals NHS. Key Responsibilities
  • Assess and develop care plans for patients with Cystic Fibrosis, working closely with multidisciplinary teams to ensure comprehensive support.
  • Provide emotional support and advocacy for patients and their families, addressing their physical and emotional needs.
  • Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
  • Develop and maintain relationships with patients, carers, and families, promoting trust and confidence in our services.
  • Contribute to service development and improvement initiatives, sharing expertise and best practices to enhance patient care.
Requirements
  • UK-recognised qualification in social work (e.g., BA Social Work, CQSW, Diploma in Social Work).
  • Significant experience in social work, with the ability to work independently and as part of a team.
  • Evidence of Continuing Professional Development (CPD) to stay up-to-date with best practices and research.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients, carers, and healthcare professionals.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing competing demands.
