Job Overview: As a Community Healthcare Professional, you will play a vital role in delivering high-quality rehabilitation services to patients in the community setting. With a strong focus on problem-solving and communication skills, you will work closely with multidisciplinary teams to develop and implement effective treatment plans.

Key Responsibilities:

* Deliver individualized occupational therapy services to patients in the community setting
* Develop and maintain effective relationships with colleagues, patients, and families
* Participate in multidisciplinary team meetings to discuss patient progress and plan further interventions

Requirements:

* Minimum 12 months NHS or Social Care experience in the community setting
* Degree in Occupational Therapy (MSc or BSc) and full registration with the Health and Care Professions Council
* Access to own car for work purposes
* Availability to commence the role within a short timeframe, committing to a minimum of three months
