About the Role

We are seeking a skilled and compassionate Mental Health Practitioner to join our team in Telford. As a key member of our Crisis Resolution Home Treatment team, you will play a vital role in providing specialist assessment and care planning to individuals experiencing mental health crises.

Key Responsibilities
  • Assess mental health crises and deliver safe and effective home treatment for identified client groups.
  • Develop and implement individualized care plans in collaboration with service users and their support networks.
  • Work in partnership with internal and external agencies to ensure seamless care coordination.
  • Contribute to the development and implementation of team goals and objectives.
Person Specification

We are looking for a Registered Mental Health Nurse with a strong background in community mental health. Essential qualifications include registration with the Nursing and Midwifery Council and a minimum of 18 months post-registration experience. Desirable qualifications include 18 months post-registration experience and previous experience working in the community.
