Patient Flow Coordinator

Job Summary:
The Discharge Hub Team is seeking a skilled Patient Flow Coordinator to manage the identification of patients, in liaison with medical and nursing staff, for a discharge/transfer pathway from other hospitals in Lothian.
Key Responsibilities:
- Manage the Home First review process on behalf of the Partnership, including daily management of waiting lists, identification of appropriate beds, and coding of patients.
- Identify and manage delayed discharges for the Partnership, with specific responsibility for coding patients under ISD National Data Recording practices.
- Provide administrative support to the department, preparing statistical information for the Discharge Hub Manager/EHSCP/Scottish Government using appropriate systems.
