About the Opportunity

We're seeking an experienced Assessment Coordinator to join our team, working closely with Sheffield City Council to promote ongoing enablement for individuals receiving care.

The Job Description:

As a trusted assessor, you will play a vital role in ensuring that care plans are dynamic, continually evolving to meet changing needs and preferences.

Key Responsibilities:

  • Conduct comprehensive assessments and continuous reviews of care needs.
  • Support providers in fulfilling their duties under CQC regulations.
  • Identify and implement the least intrusive services to boost independence.
  • Proactively address life challenges to maintain quality of life.

Requirements:

To be considered for this role, you must have:

  • A good understanding of the current legislative framework in reference to adult social care.
  • Excellent communication and interpersonal skills.
  • The ability to work effectively in a fast-paced environment.

Compensation:

The estimated salary for this role is £30,000 per annum.
