Complex Care Supervisor Job Description

We are seeking an experienced Complex Care Supervisor to join our team at Continuity of Care Services. As a key member of our community-focused care organisation, you will play a crucial role in supporting our clients with complex health needs.

About the Role:

  • Collaborate with our Care Manager to set up care packages and introduce new staff to our clients
  • Maintain familiarity with the complex needs of each of our clients to ensure continuity and consistency as per their care plan
  • Supervise, shadow, manage competency assessments, conduct observations, and mentor our care assistants
  • Work with the care coordinator to maintain shift cover within the care package team
  • Perform clinical duties of personal care, medication administration, meal preparation, and light household duties
  • Participate in the on-call rota, shared with the wider team

Requirements:

  • Experience of supporting those with disabilities and complex health needs including tracheostomies, ventilators, gastrostomy tubes, and bowel management care
  • Attend mandatory fully funded training and achieve a QCF level 3 Health and Social Care qualification within the first year of employment
  • Strong communication, excellent organisation, and problem-solving skills
