About the Role:
">We are seeking an experienced Healthcare Assistant to join our team at Central London Community Healthcare NHS Trust. The successful candidate will be responsible for providing high-quality care to patients in their homes, facilitating earlier hospital discharges and preventing unnecessary admissions.
">Key Responsibilities:

- Providing clinical and administrative support to community nursing teams">
- Working closely with GPs, hospices, and acute hospitals to ensure seamless patient care">
- Ensuring patients receive up-to-date information sheets and responding to questions constructively">
- Liaising with multidisciplinary teams to refer patients as necessary">
Requirements:
">The ideal candidate will have a Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognised equivalent course. They will also possess excellent communication skills, be able to work independently and as part of a team, and have experience working in a health or social care setting.
