Occupational Therapist Specialist

Central London Community Healthcare NHS Trust is seeking an experienced Occupational Therapist to join our dynamic Unplanned Care Team in Barnet. As a key member of our team, you will provide high-quality community-based occupational therapy services to adults with complex health and social care needs.
About the RoleThis is an exciting opportunity to make a real impact in the community and contribute to service development. You will work closely with a team of healthcare professionals, including GPs, nurses, and social care partners, to deliver coordinated and holistic patient care.
Responsibilities- Deliver specialist care to adults with complex health and social care needs, focusing on early discharge and hospital admission prevention.
- Work collaboratively with multidisciplinary teams to develop and implement effective care plans.
- Supervise and support junior staff, ensuring high standards of clinical practice.
- Take an active role in service development, applying your expertise to improve patient outcomes and support new models of care.
- Degree in Occupational Therapy with HCPC registration.
- Post-graduate training in Rehabilitation, Cognitive, or Neurological Therapy.
- Minimum of 5 years post-graduate experience, with at least 2 years in older people's services or rehabilitation.
- Strong leadership, risk assessment, and problem-solving abilities.
- Excellent communication and collaboration skills.
