We are seeking an experienced and dedicated Advanced Practice Nurse to join our team at Beacon Medical Group. This is a fantastic opportunity for a skilled professional to make a real difference in the lives of our patients.

Job Description

This role involves providing high-quality nursing care to patients with chronic diseases, working closely with our multidisciplinary team to deliver exceptional healthcare services. The successful candidate will be responsible for managing and monitoring the care of chronic disease patients, as well as providing training and support to other staff members.

Key Responsibilities:

  • Providing comprehensive nursing care to patients with chronic diseases
  • Developing and implementing individualized care plans
  • Collaborating with other healthcare professionals to ensure seamless patient care
  • Providing education and training to patients and staff members

Requirements:

  • RGN qualification and experience in chronic disease management
  • DBS clearance and nurse prescribing qualification
  • Experience of working in primary care and using clinical computer systems
  • Excellent communication and interpersonal skills

Salary: £45,000 - £60,000 per annum (dependent on experience)

Location: Beacon Medical Group, Plympton

Contract Type: Full-time
