Care Support Coordinator

Role Description
This role is ideal for someone who is passionate about delivering high-quality care support services in the community. You will be responsible for supporting the delivery of individual care plans, working closely with Care Coordinators to plan realistic and effective care worker rotas, and participating in multi-disciplinary reviews of service users' needs.
You will spend the majority of your working time in the community, visiting service users in their homes or other settings such as hospitals or residential/nursing homes. Your role will also entail some office-based administrative work, including shadowing new care workers and carrying out regular reviews of individual services.
Qualifications and Skills
To succeed in this role, you will need experience in carrying out initial risk and needs assessments, working in partnership with service users, their families, carers, advocates, healthcare professionals, and others to deliver holistic packages of care support. You will also need strong communication and interpersonal skills, as well as the ability to work autonomously and as part of a team.
