An exciting opportunity has arisen within the Frailty Team at Hillingdon Hospital for an experienced OT or PT to contribute to the team's efforts in providing high-quality care to patients.

We are seeking to expand the frailty team in line with the planned redevelopment of the main hospital site, with a focus on reducing hospital admissions and improving patient outcomes.

The frailty team covers the Frailty Assessment Unit, the short stay unit, and rapid access clinics, with the aim of providing a comprehensive therapy service to patients with frailty.

We are looking for clinicians who have a passion for frailty and the provision of therapy within acute medicine and same day emergency care, with prior experience in either emergency care or acute medicine with a frailty focus.

Main duties of the job

We work in operationally integrated therapy teams, and you would be expected to provide leadership for the frailty team in conjunction with other senior therapists, Therapy Team Leads, and Therapy Manager in continuously developing aspects of the therapy service.

To provide highly specialised therapy assessment and treatment for patients within Frailty, predominantly within short-stay patient areas including Frailty Assessment Unit, Emergency Department, Clinical Decisions Unit, RACE clinic, and Acute Medical Unit on a rostered basis.

To undertake delegated responsibilities to support the Therapy Lead in day-to-day service operation, evaluation, and development.

To provide highly specialised clinical assessment, treatment planning, and delivery of care for patients within Frailty areas.

Criteria

  • Diploma, Degree or Postgraduate Diploma in Occupational Therapy / Physiotherapy
  • Current HCPC registration
  • Practice Educators Course
  • Extensive post-registration relevant clinical experience within an NHS acute hospital including FAU, ED, short stay unit, AMU.
