Position Overview

We are pleased to announce a full-time opportunity for a team coordinator within Southern Health's Primary Care Network (PCN), offering a hybrid role that supports both service and administrative functions.

This role is innovative and designed to address the needs of the local community experiencing mild to moderate mental health challenges.

In our region, we have initiated a transformative plan aimed at improving adult mental health experiences through a program known as 'No Wrong Door'. This initiative is a collaborative effort involving local NHS entities, councils, Public Health, general practitioners, community organizations, and individuals with firsthand experience of mental health issues.

Key Responsibilities

This position is ideal for someone who can effectively coordinate tasks and provide support, encouragement, and direction to fellow team members. A passion for delivering exceptional customer service is essential.

Responsibilities include:

  • Delivering precise administrative assistance to the team while ensuring the confidentiality of patient records in accordance with data protection regulations.
  • Conducting system audits for local Primary Care Mental Health Practitioners, Commissioners, and PCNs, and supplying necessary data.
  • Accurately taking and transcribing minutes from a variety of meetings.
  • Processing referrals to the appropriate clinician and managing patient records.
  • Updating patient case notes as required.
  • Extracting data from relevant systems when necessary.
  • Compiling reports that may include graphics and tables.
  • Typing letters as dictated or provided by the team and associated PCNs.
  • Scheduling appointments within the patient record system for clinics and individual consultations.
  • Managing incoming calls and relaying messages promptly.
  • Creating tailored documents for the team, such as informational leaflets.
  • Organizing team events as needed.
  • Monitoring the quality of administrative data input.

Qualifications

Essential

  • GCSE in English and Maths at level 4/C or above.
  • RSA II or equivalent qualification.
  • Proficiency in various computer software applications.
