About the Role

We are seeking a highly skilled and compassionate Registered Professional to join our Urgent Care Access Hub and Home Treatment Team in Coventry. As a key member of our team, you will play a vital role in providing high-quality care to individuals experiencing mental health crises.

Key Responsibilities
  • Perform telephonic triages and urgent assessments of mental health needs and risk
  • Formulate plans of care in an acute crisis and provide medication home treatment
  • Deliver evidenced-based interventions that follow the cluster pathway and monitor concordance to care and treatment plans
  • Support care packages as required to meet the needs of organic patients and contribute to the review of care
  • Supervise junior staff providing care delivery and manage on a rotational basis the operational duty system
