About the Role

We are seeking a highly skilled and experienced Registered Professional to join our Urgent Care Access Hub and Home Treatment Team in Coventry. As a key member of our team, you will be responsible for providing high-quality care to individuals experiencing mental health crises, working closely with our multidisciplinary team to assess, plan, and deliver effective interventions.

Key Responsibilities

  • Perform telephonic triages and urgent assessments of mental health needs and risk
  • Formulate plans of care in an acute crisis, including medication home treatment and evidenced-based interventions
  • Monitor concordance to care and treatment plans, and provide support care packages as required
  • Contribute to the review of care and supervise junior staff providing care delivery
  • Manage the operational duty system, ensuring the team delivers roles and functions as per operational policy
