Job Description

The successful candidate will be responsible for delivering general practice nursing to the whole practice population. This includes:

  • Assessing, planning, developing, implementing, and evaluating treatment programmes that promote health and well-being.
  • Assessing, planning, implementing, and evaluating individual treatment plans for patients with a known long-term condition.
  • Proactively identifying, diagnosing, and managing treatment plans for patients at risk of developing a long-term condition.

You will also be responsible for supporting patients to adopt health promotion strategies and apply principles of self-care.

The ideal candidate will have experience of working in primary care and be registered as a nurse. You should also have strong communication skills and be able to work effectively as part of a team.
