We are recruiting for an experienced SEND Key Worker to join our team at Service Care Solutions. The successful candidate will be responsible for supporting children and young people with Special Educational Needs and Disabilities (SEND) within the Cheshire East Council.
Job Summary:
The SEND Key Worker will provide specialist support to children and young people with SEND, working closely with parents, carers, and other professionals to develop and implement individualised plans.
Key Responsibilities:
- To undertake statutory assessments and develop Education, Health, and Care Plans (EHCPs) for children and young people with SEND.
- Maintaining a caseload of children with EHCPs and attending annual reviews to make changes to plans as required.
- Co-ordinating a person-centred, outcome-focused multi-agency approach to assessment, planning, and support.
- Acting as the key worker for children and young people requiring SEND needs assessment and/or EHCP.
Requirements:
- Graduate level qualification or equivalent in a relevant field (e.g. Education/Social Work/Health related/Educational Psychology).
- Extensive experience working with children and young people with SEND across early years, schools, colleges, Local Authorities, health, or independent settings.
- Proven success in collaborative partnership working with school staff, partner agencies, and families.
- Skilled in effective multi-agency working, including Statutory Assessments.
