About the Role

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Alliance for Better Care CIC. As a key member of our community health team, you will play a vital role in empowering individuals to take control of their health and wellbeing.

Key Responsibilities
  • Work closely with General Practitioners and Primary Care Network staff to provide personalized support to individuals, their families, and carers.
  • Develop trusting relationships with individuals, giving them time to focus on their priorities and needs.
  • Take a holistic approach to support, addressing the wider determinants of health and wellbeing.
  • Co-produce personalized support plans with individuals, connecting them to community groups and statutory services.
  • Draw on and increase the strengths and capacities of local communities, enabling local organizations to receive social prescribing referrals.
Requirements
  • Understanding of the wider determinants of health and their impact on communities.
  • Knowledge of community development approaches and motivational coaching.
  • Ability to listen, empathize, and provide person-centered support in a non-judgmental way.
  • Commitment to reducing health inequalities and proactively working to reach people from all communities.
  • Ability to communicate effectively with individuals, families, carers, community groups, and partner agencies.
Desirable Qualifications and Experience
  • NVQ Level 3 Health and Social Care or equivalent qualifications.
  • Experience of working in a community development context, adult health and social care, or public health/health improvement.
  • Experience of supporting people with their mental health, either in a paid or unpaid capacity.
