We are seeking a highly motivated and experienced Social Prescribing Link Worker to join our team at Alliance for Better Care CIC. As a key member of our team, you will play a vital role in supporting individuals, their families, and carers to take control of their wellbeing, live independently, and improve their health outcomes.
Key Responsibilities- Work closely with GP practices within Park and Orchard PCN to provide personalised support to individuals, their families, and carers.
- Develop trusting relationships with individuals, giving them time to focus on what matters to them, and take a holistic approach to their health and wellbeing.
- Co-produce a personalised support plan with individuals, based on their priorities, interests, values, and motivations.
- Draw on and increase the strengths and capacities of local communities, enabling local VCSE organisations and community groups to receive social prescribing referrals.
- Work collaboratively with local agencies to raise awareness of social prescribing and its role in self-management and the wider determinants of health.
- Understanding of the wider determinants of health, including social, economic, and environmental factors and their impact on communities.
- Knowledge of community development approaches and experience of working directly in a community development context.
- Ability to listen, empathise with people, and provide person-centred support in a non-judgemental way.
- Commitment to reducing health inequalities and proactively working to reach people from all communities.
- Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies, and stakeholders.
