We are seeking a highly motivated and experienced Public Health Visitor to join our team at Central London Community Healthcare NHS Trust. As a key member of our healthcare team, you will be responsible for delivering high-quality health visiting services to clients in various community settings.

The successful candidate will have a strong background in nursing and health visiting, with excellent communication and interpersonal skills. You will work collaboratively with other healthcare professionals to ensure seamless delivery of services to clients and communities.

About the Role

This is an exciting opportunity to work in a dynamic and innovative environment, where you will have the chance to develop your skills and experience in health visiting. You will be working closely with families and individuals to provide tailored health plans and advice on evidence-based healthcare practices.

Responsibilities

- Deliver the healthy child programme and work in partnership with families to develop and agree tailored health plans as required.
- Utilise specialist skills and knowledge to advise and support individuals and families on evidence-based healthcare practices.
- Understand the caseload profile including the health needs of the defined population and local communities, targeting outreach services to meet their needs.
- Initiate referrals to multi-disciplinary/agency colleagues as required to ensure adequate care and support for families and children.
- Assess, plan, implement, and evaluate individual/group programmes of care that meet the public health needs of the population.

Requirements

- Registered Nurse
- Registered Health Visitor
- Current NMC registration
- Evidence of continuing post-registration education and training
- Recent health visiting experience
