Job Summary

We are seeking a highly motivated and enthusiastic Band 6 Physiotherapist to join our specialist falls service in Leeds. As a falls specialist, you will work as part of a team in the community setting, alongside other Physiotherapists, an Advanced Clinical Practitioner, Pharmacists, and Therapy Assistant Practitioners.

Key Responsibilities:

  • Providing comprehensive multi-factorial specialist falls risk assessments and interventions to complex high risk falls patients in the community.
  • Providing specialist physiotherapy assessments, and evidence-based and goal-orientated interventions to this client group with highly complex presentations and multi-pathologies.
  • Acting as a source of specialist advice, support and education regarding falls assessment, prevention and management to other community healthcare professionals.
  • Providing a triage role through the integrated falls referral pathway.

Requirements:

  • Demonstrates a detailed theoretical and practical knowledge of common aetiologies, pathologies, conditions and presentations involving a critical understanding of theories and principles.
  • Demonstrates innovative and specialist skills to select methods and tools to holistically assess, plan, implement and evaluate in a complex and specialised field of work.
  • Makes consistent judgements requiring complex fact analysis, interpretation and comparison of options and ensures clinical reasoning is reflected accurately in case records.
