Job Description: Community Support SpecialistWe are seeking a compassionate and dedicated Community Support Specialist to join our team. In this role, you will play a vital part in supporting individuals within their communities, empowering them to live fulfilling lives.
- Key Responsibilities:
- Provide personalized support to individuals based on their unique needs and goals.
- Develop and implement care plans in collaboration with clients, families, and other healthcare professionals.
- Assist with daily living activities, such as personal hygiene, meal preparation, and medication management.
