Job Description: Community Support Specialist

We are seeking a compassionate and dedicated Community Support Specialist to join our team. In this role, you will play a vital part in supporting individuals within their communities, empowering them to live fulfilling lives.

  • Key Responsibilities:
  • Provide personalized support to individuals based on their unique needs and goals.
  • Develop and implement care plans in collaboration with clients, families, and other healthcare professionals.
  • Assist with daily living activities, such as personal hygiene, meal preparation, and medication management.
  • Promote social integration and community engagement through various activities and support groups.
  • Maintain accurate records and documentation of client interactions and progress.

Essential Skills and Qualifications:

  • Demonstrated experience in providing direct care to individuals within a community setting.
  • Strong understanding of relevant healthcare policies, procedures, and best practices.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team in a fast-paced environment.
