Key Responsibilities
  • Creating personalized support plans with customers and coordinating our team of support workers to ensure effective implementation.
  • Encouraging customer participation in our customer involvement groups and promoting their engagement in daily living activities.
  • Providing support with daily living tasks such as meal preparation, shopping, budgeting, and maintaining tenancy, with the goal of helping customers achieve independent living.
  • Conducting risk assessments, support planning, goal setting, and regular reviews to ensure customer needs are met.

Requirements
  • Passion for supporting customers to live their best life, with a focus on collaboration and attention to detail.
  • Experience in creating person-centered support plans and supporting colleagues in working to these plans.
  • Experience in coordinating and assessing customer referrals, with a strong understanding of mental health needs.
  • Ability to work independently, remain calm under pressure, and demonstrate a resilient approach.
  • Valid driver's license and access to a vehicle insured for business purposes (mileage reimbursement provided).
