Job Title: Mental Health Nurse/Practitioner – Older Adults Community

**Job Summary:**

We are seeking a skilled Mental Health Nurse/Practitioner to join our Older Adults Community Mental Health Team. As a key member of our team, you will play a vital role in supporting the management of the waiting list for older adults with various mental health issues and co-morbidities.

**Key Responsibilities:**

* Work closely with the multi-disciplinary team and external agencies to support patient care
* Possess excellent communication skills and be aware of risk management
* Support the management of the waiting list for older adults with various mental health issues and co-morbidities
