Midwife Community Care Specialist

Job Summary
We are seeking a skilled and compassionate Registered Midwife to join our Community Midwifery Team at the Countess of Chester Hospital NHS Foundation Trust. As a key member of our team, you will provide high-quality antenatal and postnatal care to women in the Ellesmere Port, Chester City, and Chester Rural areas.
Main Responsibilities
- Assess the full range of care needs and develop, implement, and evaluate programmes of care throughout the ante-natal, intra-partum, and post-natal period in either the community or hospital setting.
- Work collaboratively with other professionals and agencies to ensure women and their families' needs are met, especially in relation to ongoing health/social requirements and discharge arrangements.
- Establish and maintain effective communication with women and their families, ensuring they are involved in the planning and delivery of care.
- Collect, collate, and report information, ensuring accurate records are maintained.
- Recognise and respond appropriately to urgent and emergency situations.

The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and Welsh patients covered by Betsi Cadwaladr University Health Board. Our services are provided from three locations: The Countess of Chester Hospital, Ellesmere Port Hospital, and Tarporley War Memorial Hospital. We employ over 6258 staff and provide acute emergency and elective services, primary care direct access services, and obstetric services to a population of approximately 407,000.
Requirements
- Registered Midwife qualification.
- Degree or relevant experience.
- Ability to communicate in a variety of midwifery settings.
- Ability to develop effective interpersonal relationships with colleagues in a variety of healthcare settings.
- Able to deal with women and their families at times of emotional stress.
