We are seeking a skilled and compassionate Community Midwife to join our team at the Countess of Chester Hospital NHS Foundation Trust. As a Community Midwife, you will play a vital role in providing high-quality care to women and their families in the community.
About the RoleThe successful candidate will be responsible for assessing the full range of care needs and developing, implementing, and evaluating programs of care throughout the ante-natal, intra-partum, and post-natal period in the community or hospital setting. You will work collaboratively with other professionals and agencies to ensure women and their families' needs are met, particularly in relation to ongoing health/social requirements and discharge arrangements.
Key Responsibilities- Work autonomously within guidelines and Sphere of Professional Practice within a designated area, performing comprehensive assessments, implementing, and evaluating care delivery according to the health and social needs of women and their babies.
- Collaborate with other professionals and agencies to ensure women and their families' needs are met, especially in relation to ongoing health/social requirements and discharge arrangements.
- Establish and maintain effective communication with women and their families, collecting, collating, and reporting information, ensuring accurate records are maintained.
- Recognize and respond appropriately to urgent and emergency situations, involving women and their families in the planning and delivery of care.
The Countess of Chester Hospital NHS Foundation Trust provides services to West Cheshire and Welsh patients covered by Betsi Cadwaladr University Health Board. Our services are provided from three locations: The Countess of Chester Hospital, Ellesmere Port Hospital, and Tarporley War Memorial Hospital. We employ over 6258 staff, providing acute emergency and elective services, primary care direct access services, and obstetric services to a population of approximately 407,000.
