About the Role

We are seeking a highly motivated and experienced professional to join our team in providing specialist care services for vulnerable adults in the London Borough of Bexley.

As a Care Specialist in Complex Care, you will be responsible for conducting thorough assessments of service users' needs, risks, and strengths, and developing and implementing personalized care plans in collaboration with service users, their families, and other professionals.

  1. Assessment and Planning: You will conduct regular assessments of service users' needs, risks, and strengths, and develop care plans tailored to their individual requirements.
  2. Care Plan Development: In collaboration with service users, their families, and other professionals, you will develop and implement personalized care plans that meet the complex needs of vulnerable adults.
  3. Regular Reviews: Regularly review care plans to ensure they remain relevant and effective in meeting the changing needs of service users.
Requirements
  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Degree level or equivalent in Social Work
  • Post-qualified experience in complex care
