Care Team Supervisor

Job Summary: We are seeking a skilled Team Leader to join our team at Acuity Care Group. The successful candidate will be responsible for supporting an individual living in Tiverton, Exeter, who has Learning disabilities, Autism, and Mental health.
Key Responsibilities:
- Provide leadership and support to the individual, promoting their independence and well-being.
- Develop and implement personalized care plans to meet the individual's needs.
- Work collaboratively with other healthcare professionals to ensure comprehensive care.
- Manage shift patterns, including 8:00am - 15:30pm, 15:30pm - 23:00pm, and sleep-ins.
- Ensure the individual's safety and security at all times.
Requirements:
- Must have Team leader experience in the healthcare sector.
- Must have experience working with individuals with Learning disabilities, Autism, and Mental health.
- Must have a full UK driver's license.
