Cancer Patient Pathway Coordinator

We are seeking a highly skilled and compassionate Cancer Patient Pathway Navigator to join our team at Tameside and Glossop Integrated Care NHS Foundation Trust. As a key member of our team, you will play a vital role in supporting patients and their families through the cancer pathway, ensuring seamless care coordination and navigation.
Key Responsibilities- Coordinate care by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent needs.
- Provide advice and refer on or sign-post to other sources of support.
- Coordinate diagnostic and monitoring through appointments and investigations.
- Coordinate face-to-face appointments for those with complex results.
- Maintain accurate patient records on both the Trust Electronic Patient Record and the Somerset Cancer Register to reflect the care given.

- Experience of customer service or dealing with service users or the general public.
- Relevant experience of working within an acute healthcare setting, particularly in administrative or coordination roles.
- Experience of working in a busy environment.
- GCSEs (or equivalent) including English and Maths at grade C or above.
- ECDL or equivalent demonstrable skills.
- NVQ Level 3, BTEC or equivalent in a relevant field.
- Skilled in communicating sensitively with patients/service users.
- Competent in the use of Microsoft Office software.
- Demonstrate ability to use an NHS Patient Administration System.
- Ability to communicate effectively with colleagues, clinicians and managers both internal and external to the Trust.
- Ability to work on own initiative and as an effective and responsible team member.
- Ability to work to agreed time scales in line with policy and guidance by organising workload to meet priorities.
- Ability to co-ordinate others in complex situations.
- Skills in data management and analysis, with a focus on accuracy and attention to detail.
