Cancer Patient Pathway Coordinator

We are seeking a highly skilled and compassionate individual to join our team as a Cancer Patient Pathway Coordinator. This is a challenging and rewarding role that requires strong communication and organizational skills, as well as a deep understanding of the complexities of cancer care.
Key Responsibilities- Coordinate care for patients with cancer, ensuring that they receive timely and effective treatment.
- Act as a single point of access for patients and their families, providing support and guidance throughout their cancer journey.
- Work closely with multidisciplinary teams to ensure that patients receive comprehensive and coordinated care.
- Enter accurate data onto the Trust's Electronic Patient Record and Somerset Cancer Register database.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
- Experience of working in a healthcare setting, particularly in administrative or coordination roles.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
- Basic knowledge of medical terminology and cancer care pathways.
- Ability to maintain accurate records and data, with a focus on attention to detail.
