About the Role

We are seeking a highly skilled and compassionate Cancer Patient Pathway Navigator to join our team at Tameside and Glossop Integrated Care NHS Foundation Trust. As a key member of our team, you will play a vital role in supporting patients and their families through the cancer care pathway.

Key Responsibilities
  • Coordinate care by providing a single point of access for patients, including rapid re-entry into the system for those with urgent needs.
  • Provide advice and refer patients to other sources of support, or sign-post them to relevant services.
  • Coordinate diagnostic and monitoring appointments, investigations, and face-to-face meetings for patients with complex results.
  • Maintain accurate patient records on the Trust's Electronic Patient Record and the Somerset Cancer Register.
Requirements
  • Experience of working in a busy environment, preferably in an acute healthcare setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work on own initiative, prioritize tasks, and manage workload to meet deadlines.
  • Knowledge of the National and Local Cancer Targets, and their implications for patient care.
