Cancer Patient Pathway Coordinator

About the Role
We are seeking a skilled Cancer Patient Pathway Navigator to join our team at Tameside and Glossop Integrated Care NHS Foundation Trust. As a Cancer Patient Pathway Navigator, you will play a vital role in supporting patients and their families through the cancer pathway, ensuring they receive the best possible care and support.
Key Responsibilities
- Act as the patient's navigator, tracking their timed pathway against national guidance.
- Support patients and their families through the relevant cancer pathway across secondary, community, and primary care as required by the patient.
- Coordinate care, education, and support for people with complex care needs and provide a point of access to patients and carers.
- Enter accurate data onto the Trust's Electronic Patient Record and Somerset Cancer Register database.
Requirements
- Experience of working as part of a team and in isolation.
- Positive attitude towards change and its implementation.
- Experience of customer service or dealing with service users or the general public.
- Relevant experience of working within an acute healthcare setting, particularly in administrative or coordination roles.
