About the Role:

We are seeking a highly organized and empathetic Personal Support Coordinator to join our care services team. In this role, you will work closely with customers to understand their needs and develop tailored support plans.

Key Responsibilities:

  • Conduct comprehensive assessments of customer needs
  • Develop and implement personalized support plans
  • Coordinate care services to meet customer needs
  • Communicate effectively with customers, families, and colleagues

Requirements:

  • Degree in Social Work or related field (or equivalent experience)
  • Excellent communication and organizational skills
  • Ability to work independently and as part of a team
  • Flexibility to work varied shifts, including evenings and weekends

Benefits:

  • Competitive salary: £25,000 - £30,000 per annum (dependent on experience)
