We are seeking a skilled and compassionate Band 6 Nurse to join our Community & RADS Team at Havens Hospices. As a key member of our team, you will play a vital role in delivering high-quality patient care and supporting our patients and their families.

Job Summary

The successful candidate will be responsible for assessing patient needs, implementing and evaluating care plans, and managing a team of staff to ensure seamless delivery of care. You will work closely with our Community Sister and Head of Community Services to develop and lead the community team, promoting effective communication and a positive team spirit.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Implement and evaluate care plans, ensuring patient safety and well-being
  • Manage a team of staff, providing guidance and support to ensure effective care delivery
  • Work collaboratively with the Community Sister and Head of Community Services to develop and lead the community team
  • Conduct appraisals and performance reviews as required
  • Ensure staff are adequately trained to deliver high-quality care
Requirements
  • First-level degree or diploma in nursing
  • Current and unrestricted practice registration with the NMC
  • Post-registration education and training up to postgraduate diploma level or equivalent
  • Full UK driving license and access to own vehicle
