Job Description
We are seeking a skilled and dedicated professional to join our team as an Early Intervention Coordinator. In this role, you will work closely with young people to develop their life skills and provide support and guidance to help them overcome challenges and achieve their goals.
Key Responsibilities
- Design and Implement Life Skills Programs: Assist young people in developing their life skills through designing and implementing life and social skills programmes in individual and group work settings.
- Run a Weekly Drop-In Center: Run a weekly drop-in center as part of our prevention strategy to offer early help and advice to young people and their families.
- Deliver Outreach Sessions: Set up and deliver outreach sessions working with a range of partner agencies and be the first point of contact for customers referred to the service.
- Develop Partnership Working: Develop and implement enhanced partnership working across the public and voluntary and community sectors to integrate the offer of housing advice, options, and assistance and encourage joint work to provide the most relevant support to customers across a range of support needs.
- Monitor and Review Joint Working Arrangements: Monitor and review joint working arrangements and feedback on outcomes and performance to Youth Hub governance structures.
- Provide Advice and Support: Provide advice and support to a range of stakeholders such as Service Heads, Social Workers, Youth Offending Service, drug and alcohol services, physical and mental health services, housing providers, etc.
- Develop and Deliver Training: Develop and deliver training/briefings to partnership agencies about the service and be an ambassador for an integrated approach to working with young people to offer early help and their wider support needs at the earliest opportunity.
- Work Closely with Other Organisations: Work closely with other organisations and agencies to provide holistic multi-agency responses to housing and support needs.
- Carry Out Homelessness Prevention Work: Carry out homelessness prevention work, including liaising with landlords, families, and others, and by applying mediation and conciliation techniques wherever appropriate.
- Manage and Prioritise Caseload: Manage and prioritise own caseload and evaluate each case, making decisions on appropriate options, meeting deadlines, and referring to other services and outside agencies as appropriate.
- Safeguard Young People and Vulnerable Adults: Safeguard young people and vulnerable adults at risk, through appropriate escalation and reporting to statutory services. Inter-agency referral and follow-up work.
- Ensure Accessibility of Early Help Service: Ensure that the early help service is accessible to young people, that all relevant parties are informed of outcomes, and that follow-up support is in place as and when required.
- Attend Panel Meetings and Review/Case Conference Meetings: Attend panel meetings and review/case conference meetings regarding young people as and when required.
- Keep Up to Date with Legislative Changes: Keep up to date with legislative changes and other new developments relating to young people and early help resources, etc.
- Service Monitoring: Submit monthly reports on drop-in service, number of sessions, and activities undertaken.
- Measure Service Impact: Measure service impact through feedback and reporting on grant outcomes.
- Capture Feedback from Schools and Other Agencies: Capture feedback from schools and other agencies.
- Report on Outcomes of Support Provision: Report on outcomes of support provision, eg. prevention of crisis, signposting, and referrals.
- Support and Encourage Young People to Undertake Life Skills Programme: Support and encourage young people to undertake the St Basils life skills programme and achieve the qualification.
- Encourage and Motivate Young People to Engage with and Sustain Education, Training, and Employment Opportunities: Encourage and motivate young people to engage with and sustain education, training, and employment opportunities.
- Support Young People's Progress: Support young people's progress, working innovatively and flexibly to assist with any problems which may arise.
- Refer Young People for Opportunities in Work Experience and Employment: Refer young people for opportunities in work experience and employment as appropriate.
- Establish and Develop Links with Training Providers, Employers, Volunteering Networks, etc.: Establish and develop links with training providers, employers, volunteering networks, etc. in order to improve access to training opportunities for young people.
- Identify and Recommend Additional Resources and Services: Identify and recommend additional resources and services appropriate to the client group.
- Maintain Accurate Records of Young People Action Plans and Progress: Maintain accurate records of all young people action plans and progress.
- Undertake Holistic Risk and Needs Assessments: Undertake holistic risk and needs assessments and develop support and risk management plans.
- Support and Encourage Young People to Develop Positive Family and Other Support Networks: Support and encourage young people to develop positive family and other support networks.
- Prepare Young People to Take Up and Sustain Settled Accommodation: Prepare young people to take up and sustain settled accommodation and to continue to provide ongoing support to them.
- Be Available to Young People for Regular Support Sessions: Be available to young people for regular support sessions and to respond to crises.
- Take Responsibility for Safeguarding the Welfare of Young People: Take responsibility for safeguarding the welfare of young people.
- Promote and Facilitate Participation of Young People in Their Individual Support Plans: Promote and facilitate the participation of young people in their individual support plans and in the management of the service.
- Facilitate Young People's Access to Other Support Services: Facilitate young people's access to, and take up of, other support services.
- Ensure Benefit Claims Are Made and Maintained: Ensure benefit claims are made and maintained in order to maximise young people's income.
- Organise and Accompany Young People to Appointments: Organise and accompany young people to appointments as appropriate.
- Maintain Accurate Records, Support Plans, Day-to-Day Records: Maintain accurate records. support plans, day to day records.
- Be Flexible by Being Available Out of Hours: Be flexible by being available out of hours on a rota basis to offer information, advice, and guidance to colleagues and in crisis situations, provide emergency cover within your own project.
- Work as Part of a Team: Work as part of a team, providing cover for colleagues as required by the line manager.
- Assist in the Preparation of Accommodation: As part of a team, assist in the preparation of accommodation, which will involve some cleaning duties.
