Senior Nurse

We are seeking an experienced Senior Nurse to join our Early Intervention Community Team within Transformation Unit. As a key member of our team, you will play a vital role in delivering high-quality care to patients in their own homes.
Key Responsibilities:
- Conduct comprehensive assessments of patient care needs
- Develop and implement personalized care plans
- Collaborate with multidisciplinary teams to ensure seamless care delivery
- Provide education and supervision to less experienced staff members
- Promote health and well-being through collaborative working with patients and their families
Requirements:
- Registered Nurse Level 1 (Adult) qualification
- Up-to-date post-registration education relevant to the role
- Degree in a relevant subject
- Nurse prescriber qualification
- Significant experience in a rehabilitation setting or care of the older adult
- Experience working in a community setting
- Comprehensive assessment skills and knowledge of patient needs
