About this role

In this challenging yet rewarding role, you'll work collaboratively with colleagues to assess patient needs, develop personalized care plans, and provide high-quality support with mobility, maintaining accurate records of patient progress and outcomes.

Key responsibilities
  • Conduct thorough assessments to identify patient needs and preferences.
  • Develop and implement comprehensive care plans, incorporating input from multidisciplinary team members.
  • Maintain high standards of evidence-based practice, adhering to relevant policies and procedures.
Requirements
  • NVQ Level 3 in Health and Social Care OR an equivalent alternative Level 3 Qualification.
  • Previous experience in a healthcare setting, preferably in end-of-life care.
  • Strong analytical and problem-solving skills.
