Job Title: Healthcare Support Worker

An exciting opportunity has arisen for a motivated and compassionate individual to join our team as a Healthcare Support Worker. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to patients with frailty in our communities across Plymouth, the South Hams, and West Devon.

About the Role:
  • You will work closely with consultants, pharmacists, dietitians, and other healthcare professionals to ensure seamless, integrated care on the virtual ward.
  • As a Band 3 HCSW, you will support the delivery of healthcare-related tasks under the direction of the wider MDT.
Key Responsibilities:
  1. Support the delivery of healthcare services to patients with frailty.
  2. Provide emotional and spiritual care to patients, promoting their comfort and dignity.
Requirements:
  • NVQ level 3 in Health and Social Care or equivalent qualification.
  • Care certificate.
  • Experience working in a Physical Health setting, preferably in an inpatient or community environment.
Salary:

The estimated salary for this role is £23,364 - £27,625 per annum, depending on experience.
